As we approach the holiday season, caring for your complex, geriatric patients to keep them well and out of the hospital is a priority. Balancing that with the expected staff shortages to accommodate requested time off poses challenges for patient coverage.
For your eligible patients, you have a partner in Landmark. Qualifying health plans collaborate with Landmark to bring supplemental care to eligible members, where they live and when they need it. Landmark is a mobile medical group, bringing care into the homes of seniors with chronic illnesses.

Some of your patients haven’t fully returned to consistent in-person visits. Landmark’s mobile care team can reach them where they live, and includes doctors, advanced practice providers, behavioral health providers, social workers, pharmacists, dietitians and nurse care managers.

Hospital capacity continues to be an issue due to COVID-19, as well as an influx of patients needing urgent care who had delayed addressing issues during the height of the pandemic. The system may be further taxed as we enter the upcoming flu season. Landmark’s in-home urgent care services help to reduce avoidable emergency room visits, hospital admissions and readmissions for our complex patients, which keeps critical beds open.

Landmark can be a partner when you and your staff are unavailable. Our call center is staffed by providers who respond 24/7, including weekends and holidays to help triage care needs and, if needed, have a provider address after-hour needs by phone, video or in the home.
