HealthTeam Advantage and Landmark Health Bring Back House Calls
High-need members get greater access to care
Greensboro, NC, February 25, 2020 – HealthTeam Advantage (HTA) has partnered with Landmark Health and its affiliated medical group (Landmark) to bring physician-led in-home medical care to Medicare Advantage members living with multiple chronic conditions.
Based in Greensboro, HealthTeam Advantage offers a personalized concierge approach to ensure its Medicare Advantage members feel like part of its family and receive exceptional customer care. Through the collaboration with Landmark Health, HealthTeam Advantage can now offer high-need members in-home medical services and care coordination, at no additional cost. Landmark services are offered to HTA members who have multiple complex chronic medical conditions (such as heart failure, chronic kidney disease or malnutrition).
“We refer to these members as our most vulnerable and have asked Landmark Health to partner with us to bring doctors, nurse practitioners, and other health professionals to members’ homes to help keep them well and out of the emergency room and hospital,” says Dr. David Grapey, HTA’s chief medical officer. “Helping our members get the care they need, when they need it, is one of our guiding principles. Landmark provides front-door access to routine visits, urgent medical care, condition education, medication support, social support, and more.”
HealthTeam Advantage recognizes the need for specialized care for patients with chronic conditions as they can have unique challenges (such as being homebound or home-limited) that make it difficult to see doctors as often as needed. Instead, these patients often go to emergency departments, which can lead to unnecessary hospital admissions and skilled nursing facility stays. Landmark has reduced unnecessary ED admissions 39 percent for engaged patients.
“Landmark is one of the largest risk-bearing provider organizations caring for complex populations in the United States,” says Nick Loporcaro, Landmark’s chief executive officer. “We attribute our growth to our focus on meaningful outcomes for our patients. Our patients are living longer, have high satisfaction with our services, and save healthcare dollars by staying home instead of going to the emergency department. We’re excited to bring our 24/7 clinical care model to HealthTeam Advantage members.”
HealthTeam Advantage has more than 15,000 members in the Triad and it looks forward to working with Landmark to bring proactive and on-demand care to more North Carolinians in their homes.
